Register as a New Patient

New Patient Registration

We are currently accepting patients within our practice area, please view our Practice Boundary page to see if you live within our practice area. If you are unsure, please ask the reception staff if you are in our catchment area.

To pre-register please download the following forms, and bring them in to the surgery signed with the necessary documentation (see below).

If you identify as non binary please add in your title on the registration form as Mx, if you do not wish to give a title please leave this blank.

The ideal times to come to St Marks Medical Centre are between 11:00 – 14:00 as these are quieter hours. Coming to St Marks outside these hours may result in you waiting as clinics will be running, evenings are particularly busy especially Friday nights. If you need to come in the evening due to commitments we would recommend you come on Mondays between 18:30 and 20:30.

Documentation

Anyone in England can register with a GP surgery. We will ask you for proof of your ID and address to make the registration process accurate and easy but you do not need proof of address or immigration status, ID or an NHS number.

You might be able to register with a GP surgery that’s not in the area you live. Find out about registering with a GP surgery out of your area at www.nhs.uk – Registering with a GP surgery outside the area you live.

Registration Appointments

Registration appointments are most often booked a week after you hand in the registration forms. (If you require an appointment straight away, please speak to reception regarding your situation and they will advise).

We will then make an appointment which will initially be by phone to speak to the Practice Nurse for a new patient health check. This will make sure that our records are accurate and all your routine health checks are up to date. Any health information held by your previous GP will also be sent to us as your new practice but this can take up to six weeks to arrive.

Patients will not be officially registered until they have attended the nurse appointment. Any cancelled appointments that are not made within the next week after the initial appointment may result is the de-registration of the patient.

If you are on any repeat medication please make the nurse aware with a list of your prescribed drugs when you attend your new registration check.

Registration Notes

Each GP practice has an area map to show which patients it takes on.

If you live within the blue area you can register with the practice.

You can still register if you live outside our catchment area, but this would be an out of area registration which means that the practice do not contractually have to provide home visits.  Please speak to one of  the reception staff for more details.

If you are currently registered with the practice and you move outside the practice catchment area you may  continue to be registered if you move to a new home. Your continued registration is at the discretion of the practice and will depend on an assessment of the clinical risk of living not within the immediate catchment area. Temporary Registration

If you are ill while away from home or if you are not registered with a Doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient please contact the practice.

Please note it is up to the discretion of the practice to register you as a temporary patient. You will need to come into the practice with the necessary documentation to sign the forms before you will be registered with the practice for temporary services.

Register with us online

The registration process will take around 15 minutes to complete. You will need to complete the process in one session. You do not need your NHS number, but it could make registration easier.

You may be asked for:

  • details of previous GP surgery
  • basic health and medical information

The information you provide will be used to identify the correct medical records. It will also help the GP surgery to offer relevant and appropriate healthcare services.

Using Online Services

Depending on your choice of app, you may be asked to link your app to your GP practice using a linkage key, ODS code and account ID, please fill out the form below and send it back to the practice.

For security reasons we will also need to verify your identity so we advise you to come to the practice with your filled in registration form and an ID so one of the staff can verify you. Alternatively, you can email us a copy of your ID along with your completed registration form to [email protected].

Once we recieve confirmation of this we can complete your registration and provide you with the required connection details.

NHS App

You can self register for the NHS app for the following:

Get your NHS COVID Pass – view and share your COVID Pass for event trials in England and travel abroad.
Get advice about coronavirus – get information about coronavirus and find out what to do if you think you have it.
Order repeat prescriptions – see your available medicines, request a new repeat prescription and choose a pharmacy for your prescriptions to be sent to.

MY GP App

myGP app is a patient facing service for smartphones. The app is designed to improve patient access to primary care services, to enable safe and secure communication between the practice and its patients, support data collection and to encourage self-care through monitoring and medicines adherence. It allows anywhere, anytime access – 24 hours a day, 7 days a week, at home or abroad.

Core functionalities of the current version of the myGP are:

Registration takes just a few minutes. You need to download the app on to your android or iOS device, then register using your date of birth and mobile phone number for authentication (you must have your mobile number registered with the practice for the app to authenticate your registration). Once this is done you will be sent a text message with an authorisation code.
