National Data Opt-out Policy
The national data opt-out allows a patient to choose if they do not want their confidential patient information to be used for purposes beyond their individual care and treatment – for research and planning.
Please see below link to NHS UK explaining about the national data and research planning initiative. You will see an option ‘Make Your Choice’ which will explain your options of how you choose your data to be handled and go through the opt out process. www.nhs.uk/your-nhs-data-matters.
This is the most secure way of making your choice. However, you can download and fill out the form blow which you will then need to send to the practice through one of the following methods.
- Submit an administrative request via e-consult and attach your completed form. St Marks Medical Centre – eConsult
- Send via post to the practice addressed to St Marks Medical Centre, 24 Wrottesley Road, London SE18 3EP. Hand in to reception Register your Type 1 Opt-out preference.docx
